Case Manager – Full Time

Position is to provide transitional case management through the correctional facilities of southwest Virginia to individuals living with HIV/AIDS or at high risk of contracting HIV upon release.   Responsibilities include:   intake; service plan development; discharge planning; and other client level services.  Requirements include:  Bachelors in human service or related field with one year experience working in correctional facilities or with HIV/AIDS or Associates in human service or related field with three years experience working in correctional facilities or with HIV/AIDS.  Mon-Fri 8am – 5pm – occasional nights and weekends; reliable transportation for travel within southwest Virginia region; and must pass criminal background check for entry into correctional facilities.  Send cover letter, resume and three references to:  Pam Meador, Council of Community Services P.O. Testing Specialist – Full Time

Position to provide HIV counseling and testing to a diverse population within correctional facilities – Mon-Fri 8am – 5pm with occasional nights and weekends.  Requirements include:  High school graduate; clean criminal background; must be 21 years of age or older; and have reliable transportation.  Training in HIV field a plus.    Send cover letter, resume and three references to:  Pam Meador, Council of Community Services Pam Meador, Council of Community Services P.O.
