Medical Case Manager 

FT position available to provide medical case management and supportive services to individuals living with HIV and/or HCV. Occasional nights and weekends – must be 21 years of age, with minimum Bachelors’ in Human Services or related field or related experience for a period of two years. Position requires work within the Department of Corrections on occasion. Must have reliable transportation. Candidate must pass all Local/State/Federal Criminal Background checks. Non-Medical Case Manager/Navigator 

FT position available to provide non-medical case management; housing navigation; and supportive services to individuals with HIV and/or HCV. Occasional nights and weekends – must be 21 years of age, with minimum Associates in human service or related field and minimum of one years’ experience in case management and housing. Position requires work within the Department of Corrections on occasion. Must have reliable transportation. Candidate must pass all Local/State/Federal Criminal Background checks. Project Testing Specialist 

FT position available to provide HIV/Hepatitis education, counseling and testing services to diverse populations and venues, including within correctional facilities, drug treatment, bars, colleges, etc. Occasional nights and weekends – must be 21 years of age with minimum of HS diploma, Human Services experience or Knowledge of HIV/HCV preferred. Reliable transportation a must. Wage range $28,500 – $30,000 plus excellent benefits. Contact Specialist 

FT position available for contact specialist to provide health and human services information and referrals to Virginia residents via phone, e-mail and in person. Must be able to uphold a high standard of professionalism in a fast-paced contact center and deliver exceptional service to diverse callers seeking a variety of assistance. Excellent customer service and telephone skills are required. Knowledge of health and human services strongly preferred. Understanding of basic computer functions is required. Applicants must be available from M-F 2– 10:15 p.m., some weekend and evening shifts and occasional holidays.
